Apply to become a Host!

This form is the second stage of registering your interest in providing a home and support for a person with a disability in your community. It’s designed for people who have already found out about what it means to be a Host, and are now ready to take the next step. If you’re still at the early stages of finding out about what it means to be a Host, we suggest you go to Request Information Form before proceeding with this step.

Roadmap

Here are the four stages involved in becoming a Host for a person with a disability:

stage 1

This form has 4 pages and may take at least 10 minutes to complete. If at any point you'd like to pause and resume the form at a later time, you can click on the save button, create a password and, if you choose, you will be able to log in later to complete an application form to proceed to the next stage in becoming a host or a housemate for a person with a disability. You won’t have to re-enter details you’ve already provided.

    About You/the Host

    Thinking about the person who is likely to play the main support role for the person with a disability in your home (the “host”) – we’ll refer to this person as “you”.

    Depending on the needs of the client, do you think you would be able to:
    Assist with meals and other help in the home(Required)
    Provide assistance in getting dressed(Required)
    If they needed this, and if you were trained in safe manual handling, assist them to get out of bed/into a wheelchair or similar?(Required)
    If they needed this, and if you were appropriately trained, manage medication on their behalf and record medication administered?(Required)
    Help with transport e.g. to a community activity (i.e. drive them)(Required)
    Accompany them from time to time to participate in community activities(Required)

    If you have preferences relating to the person with a disability you might host in your home, let us know about those here. Note that we will use these as a guide only and you will have the opportunity to meet a potential client for your home before having to make any commitment.

    What happens now?

    We will review your Application and start considering whether we already have or can find a Client in your area who might be a match.

    • This could be a quick process, especially if you are applying with a specific Client in mind and/or if we’ve already spoken with you about a specific opportunity.
    • Otherwise, it might be some time before we identify a potential match in your area. In this case, we will email you periodically with any updates. When you receive those emails you will also be able to de-register if you wish.
    stage 2

    If we are close to matching you with a potential Client, we may also offer you the opportunity to start your final onboarding process. If so, we will contact you with some next steps, including:

    • a link to a site where you can upload some documents.
    • a login for a training site where you can complete some online training courses if you’re ready.
    • a pro-forma contract (a contract that’s not ready for signing but gives you an idea of what will be in the contract you will need to sign when we match you).

    Whatever our next steps with your application, we really appreciate your willingness to be considered as a host for someone living with a disability in your community.
