My Supports delivers four core services with a community-first approach, drawing on our lived experience and innovative thinking to provide safe, reliable support, enhancing independence for individuals with disabilities.

Our local Neighbourhood Teams help build connections within the community. We offer a range of services, including Advice and Support Coordination, Home Support Services, Home and Living Options, and Community Participation, designed to help individuals achieve their goals.

Advice & Support Coordination

A Neighbourhood Team Leader or Support Coordinator is your key adviser, communicator, coordinator and problem-solver. They’ll help you set goals, find the right local supports, and think about your long-term future.

We're experts in flexible, independent living alternatives, not just standard group homes. We can help you live where you want to live, with whom you want to live, and support you to increase your independence, even if you don't have NDIS home and living funding.
